Media and Marketing Agreement

I hereby grant Peachy Chic Cosmetics the full right to take, publish, and reproduce photographs of me, my face, my eyes, my eyebrows, and/or eyelashes both before and after this procedure. I understand and that these photographs may be used for advertising, education, or other purposes. I grant Peachy Chic Cosmetics the right to use and retouch these photographs as deemed necessary by Peachy Chic Cosmetics. I further expressly assign any copyright in these photographs to Peachy Chic Cosmetics. I also grant my consent for Peachy Cosmetics to use my image and likeness as contained in these photographs for any advertising or other purposes.

I have read and fully understand all information in this agreement, and realize that this consent agreement is legal and binding.
